What are therapy progress notes?
Therapy progress notes are clinical records documenting what happened during a session — the client's presenting concerns, clinician observations, interventions used, and the plan for next session. Required for licensing, insurance billing, and continuity of care. Common formats include SOAP, DAP, and BIRP.
How long does it take to write a therapy progress note?
