FAQ

Progress note questions

What are therapy progress notes?

Therapy progress notes are clinical records documenting what happened during a session — the client's presenting concerns, clinician observations, interventions used, and the plan for next session. Required for licensing, insurance billing, and continuity of care. Common formats include SOAP, DAP, and BIRP.

How long does it take to write a therapy progress note?

Most therapists spend 10–20 minutes writing a single progress note. For a caseload of 20–30 clients, that's 3–10 hours of weekly documentation — much of it after hours. Notelyx reduces this to under 5 minutes per note by generating a draft from bullet-point observations.

What is the difference between SOAP, DAP, and BIRP notes?

SOAP has four sections: Subjective, Objective, Assessment, Plan. DAP has three: Data (combining S and O), Assessment, Plan. BIRP has four: Behavior, Intervention, Response, Plan — with explicit focus on what the therapist did. All three are widely accepted by insurers.

Are AI-generated therapy notes legally valid?

Yes, provided the therapist reviews, edits, and signs the note. AI generates a draft — the licensed clinician remains responsible for accuracy. Notelyx generates a draft you review and sign, which locks the note with your name, license number, and a timestamp.

How do I keep therapy notes HIPAA compliant?

HIPAA compliance requires encrypted storage, controlled access, audit trails, and a BAA with any third-party software processing PHI. Notelyx meets all requirements: data is encrypted at rest and in transit, raw observations are deleted after 24 hours, and every user receives a signed BAA.
